When you're hired, you may enroll in as many of Mohawk's benefit plans as you choose. With a few exceptions, you are eligible for benefits after 90 days on the job. No payments are deducted from your check until you have been with the company for 90 days.
Because we recognize that all employees have changing needs, we offer you an opportunity to change your benefit coverage during December of each year. You can return an enrollment form to your benefits administrator or human resources department during December if you want to add, drop or change your coverage.
If, at any time during the year, you have a change in family status, you may change your coverage. A change in family status includes birth, adoption or guardianship of a child; marriage, divorce or legal separation; death of a spouse or child; your spouse taking a job or leaving a job; changing from full-time to part-time employment for either you or your spouse; or becoming eligibile to receive Medicare, Medicaid or Peachcare. You need to complete paperwork with your benefits department or human resources department in order to change your coverage. You must make the changes within 30 days of the change in family status.
Mohawk offers many great benefits. Here are the highlights of some of the plans:
Mohawk offers an excellent medical plan at a competitive rate. The company pays for two-thirds of your premium, and the remaining portion is deducted from your paycheck each week. The popular plan features a low co-payment on office visits. Single coverage, employee plus one dependent coverage and family coverage is available.
If you enroll in the medical plan, you can use the prescription card to simply go to your pharmacist and pay a low percentage of the total cost. There's no paperwork to complete unless you use a non-participating pharmacy.
You can choose from either the Gold or Silver plans and can choose either single coverage or family coverage. This is a direct reimbursement plan. All you have to do is pay your dentist or optometrist and then file a one-page form for repayment. The Gold plan offers a maximum benefit of $1,500 for dental and $200 for vision per person. The Silver plan offers a maximum benefit of $600 for dental and $100 for vision per person. You can choose the coverage that's right for you.
Mohawk pays for $10,000 worth of life insurance for you. This coverage is automatic. You don't have to complete any paperwork to qualify. For a small weekly premium, however, you can carry additional life insurance on you, your spouse and/or children. The prices are low because Mohawk qualifies for group rates. Premiums vary based on age and amount of insurance selected. The insurance also features a plan through which you can receive up to 50% of the benefit if you or a covered member of your family is told by a doctor that you have less than one year to live.
After you have been employed at Mohawk for one continuous year, you qualify for participation in the company's retirement or 401(k) plan. This is a great way to save for your later years. You do not pay taxes on the money you contribute to the plan the company will match your contribution at $.50 per $1.00 for the first 4% of your wages contributed and $.25 per $1.00 for the next 2% of your wage contributed. Here's how it works:
If you earn $20,000 a year and you decide to save 4% of your pay, that's $800 per year. If you save it in a bank account, you end up paying taxes on the money and the interest the money earns that year. If you save the $800 in the 401(k) retirement plan, Mohawk will match 50 cents per dollar. Your $800 saved becomes $1200! PLUS, your W-2 form will show earnings of $19,200 in taxable income, so you will pay less in taxes.
The company also makes periodic profit sharing contributions to the 401(k) plan. Even if you don't have an account, this money is invested in your name. It's one more way the company wants to help you have a brighter future.

You will be mailed an enrollment form automatically after you have been with the company for one year.
If you are injured or become ill, the company will provide you with Short Term Disability income to help you after the eighth day of you are unable to work. You can either receive the basic coverage at no cost, or you can pay a weekly premium to increase that coverage by $50 or $75 for each week you are unable to work. Long Term Disability If, after 13 weeks, you remain unable to return to work, you will be eligible for
You will receive 50% of your hourly wage up to a maximum of $950 per month. A low weekly premium can be conveniently deducted from your check if you choose this coverage.
After one year, employees receive one week of paid vacation. After three years, employees are upped to two weeks of paid vacation. After ten years of employment, employees have three weeks of paid vacation. Employees with 25 or more years of service will receive four weeks of paid vacation each year.
The company recognizes seven holidays. Employees who have been with the company for 90 days will receive payment for these holidays.

Mohawk understands the importance of the family, and, by offering funeral pay, we hope we can help you in a small way during time of grief. Mohawk will provide up to three days of pay for time you miss from work to the death of an immediate family member.
Employees can request up to thirteen weeks of unpaid leave due to a birth or adoption of a child, illness of a relative or other qualifying personal situations. Anyone needing to request leave should call or visit their benefits administrator or human resources department.
